During the past 12 months, not counting times you went to an emergency room, how many times did you go to a doctor, nurse, or other health professional to get care for yourself?
Not including psychiatrists and other mental health professionals, is there a particular doctor, nurse, or other health professional that you see most often?  
What kind of health professional do you see most often? Do you see a doctor, nurse, or some other health professional?  
How often did they involve you in decisions about your health care as much as you wanted?  
During the past 12 months, did you use any complementary, alternative, or uncoventional therapies such as herbal supplements, acupuncture, chiropractic, homeopathy, meditation, yoga, or Tai Chi?
